Brecon Medical Group Practice, Ty Henry Vaughan, Brecon, Powys LD3 8AH is looking for an enthusiastic Practice Nurse to join our dynamic, committed and friendly nursing team.

We are a long established but modern, friendly training practice with a patient population of approx. 15,600 patients.  The clinical team includes, GP Partners and salaried GPs, Registrars, Physician Associate, Lead Practice Nurse, Triage Nurse, Practice Nurses, Health Care Assistants, Phlebotomists and Pharmacy Team. 

We have the full support of our management and non-clinical team including dispensing, reception, secretarial and facilities. Together we are committed to providing an excellent service to our patients and happy working environment.

Job Description

  • Reports To: Lead Practice Nurse
  • Hours: 37.5 (over 5 days) or 36 (over 4 days)

Annual Salary for full time (37.5hr) General Practice Nurse £30,933 to £37,601 however a higher salary may be offered to reflect experience and qualifications in general practice.

Job Summary

The post holder is responsible for the delivery of general practice nursing skills to the whole practice population as part of patient centred care.

The focus of the role is:

  • The delivery of acute care in a treatment room setting.
  • The provision of evidence based preventative care as well as opportunistic screening.
  • Support for patient’s management of long-term chronic disease, monitoring and review of health conditions including anticoagulation.

As an autonomous practitioner, the nurse is responsible for the care delivered, demonstrating critical thinking and skills in clinical decision-making in the management of patients. Working collaboratively within the dynamic general practice team to meet patient’s needs, support the delivery of care in accordance with policy and procedures.

Key responsibilities

  • Prioritise patient care, recognise potential problems and act appropriately to assist
    the patient in complex, urgent or emergency situations, including initiation of
    effective emergency care.
  • Assess, plan, develop, implement and evaluate treatment programmes that promote
    health and well-being.
  • Screening including opportunistic screening: BP checks, height, weight, smoking
    status, urinalysis, venepuncture, STI screening, cervical cytology.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of
    developing a long-term condition as appropriate.
  • Chronic Disease Management: Assess, plan, implement and evaluate individual
    treatment plans for patients with a known long-term condition. Work with other
    health care professionals to diagnose, monitor, manage and treat long-term
    conditions, including non-drug-based treatment methods in line with national and local
    policies and practice needs.
  • Support patients in order to support adherence to prescribed treatments and more.
